How do I order a form?

Ordering is very simple. Just find the form you are looking for, click the "Add to Cart" button. This will add the form you selected to your shopping cart. You will then be taken to a "Shopping Cart" page containing the item you selected. At this point, you can either continue shopping for more forms or complete the purchase of the forms in your cart.

To complete the purchase, click on the "Checkout" button at the bottom of your shopping cart page. You will then be asked to complete payment information. After a final confirmation screen, your order will be processed and completed. You will be taken to a "Thank You" page which contains the download links for the forms that you ordered as well as instructions and a receipt.

 

Can I order by phone or fax, or pay by check?

We accept payment using any major credit or debit card. We also accept payment through Paypal and Google Checkout. Currently, we do not accept orders via phone or fax.

 

How will I receive my form?

After you purchase the form, you will be taken to a download page that will contain links where you can immediately access the form and save it to your computer. You will also receive an email with the same download link.

When will I receive the forms I purchased?

Immediately. As soon as your payment is processed you will be taken to a "Thank You" page which contains the download links for the forms that you ordered. This page also contains instructions and your receipt. We will also send you an email containing links to the forms your purchased.

 

How do I download the form?

As soon as your payment is processed (which generally happens within seconds) you will be taken to a "Thank You" page which contains the download links for the forms that you ordered. The download links will allow you to download the file containing the forms you ordered and save it on your own computer.

When you click on the link, a "File Download" window will pop open and you will be given the option to "Open" or "Save" the file. Select "Save" and another window will open allowing you to decide where to save the document file you will be downloading. Select the location you want the file saved (good options may be "My Documents" or "Desktop").

In case you closed the "Thank You" page before you downloaded your forms, you will also receive links to the forms you ordered via email.

If you need the form in a different format such as Microsoft Word (.doc), Adobe PDF (.pdf), or Rich Text (.rtf), please contact customer support and request it. There is additional charge.

 

I have Acrobat Reader and my Adobe (.pdf) form still won't display.

If you can't open our Adobe Acrobat file, please check what version of Adobe Acrobat Reader you have on your computer. We recommend upgrading it to the latest version. download Adobe Acrobat for free.

 

When I try to open the form, the computer says there is an open dialog window and to close it before continuing. What is this?

If you encounter an "open dialog box" window, more than likely you do not have Microsoft Word installed on your machine. You will either need to use another program such as WordPad or request a different file format such as Adobe (.pdf).

 

I downloaded the form, but I can't open it.

This is probably due to:
i) the file format you choose is not compatible with your software or
ii) the file was corrupted when downloading.

What formats are the forms available in?

The forms are available in Microsoft Word format (.doc) and Adobe Acrobat format (.pdf). Some of our forms are also available in Rich Text (.rtf) format.

The Microsoft Word (.doc) format opens on PCs with Windows and either Microsoft Word or WordPad. On Mac, you will need Microsoft Word for Mac or AppleWorks to open this type of file. Text Edit on Mac OS X can also open these files. The benefit of using Microsoft Word files is that you may edit the form on your computer.

The Adobe Acrobat (.pdf) format works on both PCs and Macs. You need the free Acrobat Reader software to view these files (or the Preview Application included in Mac OS X). The benefit of Acrobat (.pdf) files is that they look and print identically on all computers. However, our Acrobat files are not editable. You may only print them.

Rich Text Format (.rtf) is a simple text format that supports simple effects such as bold type. This format will open on any PC or Mac, but is more difficult to format.
